Please include your total monthly income from all sources, including: salary, self-employed income, dividends and additional income from interest. If retired, please include pensions, SSi, 401k, Annuities, etc. Enter as a whole dollar amount with no decimals or characters.

The undersigned individual, recognizing that their individual credit history may be a factor in the evaluation of the applicant, authorizes MMLJ, Inc. to obtain and use a consumer credit report on the undersigned for the purpose of evaluating creditworthiness in connection with this commercial transaction.

By submitting this credit application, you authorize MMLJ, Inc. and its affiliated lenders to obtain your credit report and verify creditworthiness in connection with your loan application. You understand that this will result in a inquiry on your credit report. 

The consumer also authorizes any lending or funding service identified at the time of referral to obtain a separate consumer credit report as needed in connection with the credit evaluation and review process. This authorization is valid only for the specific purposes stated, and new consent will be required for any subsequent credit report requests. The consumer retains all rights under the Fair Credit Reporting Act (FCRA).
