RAMQ configuration request - Family physician identity

This form must be completed in order to consult in Omnimed the information related to the registration of patients with a family physician, a delegated professional or a group of physicians. This information, displayed in the Omnimed administrative center, allows you to verify whether an insured person is registered with a professional and, if so, to know their identity.

If you already have access to your institution's RAMQ registrations in Omnimed, you do not need to complete this form.

Prerequisite: Complete the Demande d’accès aux services en ligne – Identité du médecin de famille – B2B (4444) form to obtain an AIP code and associated password (to be provided below).
