Historical usage information authorization

David Energy Supply, LLC is required that customers grant their signed authorization before the company request historical usage information from utilities. With Customer signed authorization, David Energy Supply, LLC (“DES”) shall obtain up to 24 months of restricted monthly billing information, usage and hourly interval data, if available. Such information shall be used for the purpose of generating historical energy usage reports and determining the cost of energy supply for the relevant accounts. This authorization shall be valid for thirty (30) days from the date of Customer consent below.

I authorize David Energy Supply, LLC to obtain up to 24 months of restricted historical usage information from my distribution utility (either power or gas). Delivery of a copy of this authorization by facsimile or as scanned attachment to an email is good and sufficient delivery. By my checking this box, I affirm that I have the necessary corporate and legal authority to make and sign this authorization on behalf of my company or organization.*
