In submitting this rebate form, you acknowledge and agree to the following Terms and Conditions: EGR USA offer is valid only with eligible U.S. and Canada purchases during valid rebate offer periods. Offer valid while supplies last and no substitutions or rainchecks. Offer may not be combinable with all other offers or discounts available from EGR USA. Request form must be fully completed and include a copy of dated Proof of Receipt and Purchase Order Confirmation for purchase made directly from EGR USA or an EGR Authorized Online Retailer for qualifying product purchased. Any claims postmarked after 30 calendar days, or entered online after July 31^st will not be processed. The sunglasses will not be sent if the terms and conditions are unmet. Please allow 8-10 weeks for sunglasses to arrive. To qualify for this rebate, you must be a legal U.S. or Canadian resident 18 years of age or older. Only actual purchasers of the qualifying products may participate in this rebate program. Requests from groups or organizations will not be honored. Void where prohibited or restricted by law.
