Please Note: If you are applying for the benefits outside the start of the FBT year, 1st April, we will pro-rata your benefits for the remaining pay periods till the end of the FBT year (31st March) to maximise your benefits unless stated otherwise.

Annual employee fees (ex GST)

Fees are deducted from your pre-tax salary at the payroll level, thereby increasing the tax deductibility of your package.

• Everyday living benefit (Max $15,900) and Meal, entertainment & holiday benefit ($2,650) management fee: $286 total ex GST
• Please note: If you don't have any of the listed expenses, please apply for a Beyond Bank Everyday Living Visa card to utilise this benefit. An additional card fee of $2.00 per fortnight - $52.00 total ex GST will apply.

I declare that,

• I am eligible to enter into a salary packaging arrangement within the policy guidelines specified by my employer
• provided true and correct information in connection with this form;
• I will provide all documentation to support expenses claimed in a timely manner, and respond promptly to any reminders sent by National Salary Packaging to provide that documentation;
• I understand that if the supporting documentation is not provided, my salary package will be reduced and/or suspended until I provide that documentation;
• I understand that my salary package will not be “back-dated” for any reduced amounts;
• I accept all responsibility for any FBT liability or income tax payable as a result of not supplying supporting documentation as required; and
• I accept that my employer and National Salary Packaging shall not be responsible for any tax liability due to me not supplying supporting documents or any other non-compliance by me with the taxation laws.
• I acknowledge that I have read the internal policy guidelines (Employee benefits) outlined by my employer and agree to comply with the conditions as contained in the policy and the terms and conditions stated herein
• I understand that I can adjust my salary and benefits at any time and that charges may apply
• I acknowledge that on cessation of my employment or salary packaging arrangement, adjustments will be made to my entitlements, taking into account any liabilities that may arise
• I acknowledge that where a change in taxation rules occurs, the effect of those changes may be applied to my package
• In relation to any leased vehicle, either novated or associate, I remain responsible for any vehicle obtained under these lease agreements. This includes responsibility for finance arrangements, funding costs, and any outstanding liability at the completion of the termination of the lease.
• All reimbursement claims must be substantiated with the relevant documentation and declarations as required.
• Salary packaging could reduce your taxable income but may affect the way the ATO calculates your HECS/HELP repayments. To avoid any possible shortfall at tax time for unpaid HELP repayments, you need to let your payroll department know. You should seek financial advice if you're unsure.
