This form is designed to streamline the process of having the CoVet support team help you create or improve a custom template.

We appreciate you filling it out in a thoughtful way - providing organized information here will help speed up the template creation process. Once you've submitted the form you should receive an email within 1-2 business days to start the process.

This form is designed for you to describe one template request at a time. If you would like multiple templates created please complete the form once for each request. There is a link provided to start the process again on the submission confirmation page.

Is this request for a new template, or to improve an existing one?*
You do not need to complete this part of the form if we're creating a new template. If you would like to modify an existing template you can view this article for instructions on how to get the sharing link or code for that template.

Required Documentation

Step #1 - Submit a Blank Outline

Please provide a document that is a blank outline of the final report you hope to achieve through this template. It should feature only headers and other labels or placeholders. The goal here is to help us set up the structure of your document.

Step #2 - Submit an Outline with Comments

Provide a second document that follows the same outline, but also has some context added for each header/placeholder. This context could be a general description of what information you want to show up in a given area, what specific formatting you would like for that area, if there is default normal text you would like used in that area, etc.

Step #3 - Add Real Case Examples (Optional)

In addition to uploading the documents above, providing real-world examples can be helpful to our team in designing your template. Feel free to provide them with or without additional comments.
