Mascot appearance requests must be submitted at least two weeks prior to your event. Appearance fees may apply. Completion of this form is a request only and does not guarantee an appearance. You will be contacted to confirm details of your request. Booster reserves the right to depart any appearance deemed unsafe to performer or mascot suit without refund.
