Wealth Screening Order

Use this form to order and authorize a wealth screening in connection with your Generis consulting engagement. This form authorizes us to transmit your data to Donor Search, a third-party subprocessor, subject to the wealth screening Data Processing Agreement. Your data is kept secure and confidential consistent with our Data Protection Policy, but we just want you to be informed that we're using a third-party service.
