VAC Disability Assessment Inquiry

Prior to scheduling a Disability Assessment, you need to apply to VAC for a disability benefit. 

To start this process you can:
1. Connect with your Case Manager, or
2. Contact a Legion Command Service Officer, or
3. Login to your My VAC account

If you have any question of concerns, please reach out to our Navigation team at 778-401-9276 or reception@vfrh.ca


Once your initial application is completed, you will receive the following documents from VAC:

1. Disability benefit assessment letter from VAC
2. Medical questionnaire forms (auto pre-filled with some of your personal information)

Our team requires these 2 forms before scheduling you appointment.
