Invoice Submission Form

Main/billing contacts must submit invoices within the first two weeks of each month.

Please use the following naming conventions for invoices: Company Name_School Acronym_Month (Abbreviate)_Year

Example: Demo Company_MyCA_OCT_2023
File Name MUST be in this format: Demo Company_MyCA_October_2023
Use the same naming convention as the Invoice Name: Demo Company_MyCA_October_2023
