By completing this form, you are submitting 1 of 2 forms to apply for the Thompson Okanagan Biosphere Commitment Program. This form should completed by an authorized business decision maker, such as an owner, general manager, or other principle.

After your application is reviewed, you will be accepted into the program and an invoice of $519.75 ($495  + GST) for the annual program fee will be sent to the accounts payable email address provided below.

By submitting this agreement, you agree to pay the annual membership fees in full. The membership fee is non-refundable. Following the initial one year membership period, this agreement shall automatically renew by consecutive one (1) year periods, unless either party provides written notice to the other of its intent to terminate the agreement at least thirty (30) days prior to the renewal date.
