Requirement for New Registration FormAs per our terms and conditions, you are required to complete a new Registration Form agreeing to new Terms and Conditions and make the first monthly instalment on the new payment plan if applicable. The transfer fee will be deducted from the previous payments already made and any remaining credit balance will be applied to your new registration. The payment plan will be adjusted accordingly from the second payment onwards.
Terms for Requesting a TransferI understand that my transfer is conditional upon:- space availability in the requested course- payment of all applicable transfer fees- payment of any outstanding amount owing for registration- my acceptance of the Terms and Conditions of the new programThe CHEK Institute will contact me to confirm or deny my request for transfer and if denied, will provide me with other options.If I am selecting to convert the amount paid to date into credit, I understand that this amount will be held by the Institute for a period of 1-calendar year from the date of submission of this form, after which time it will expire. The credit will be in the form of a gift card emailed to the student. If i received any free product with my initial registration, the normal retail price of such product(s) will be deducted from the amount of tuition paid to date. The remaining balance will be the amount of credit received. If I requested the transfer fee to be deducted from the amount of tuition paid to date, then the remaining balance will be the amount. I may apply this credit towards purchases of any CHEK Institute products, courses or registration into an Advanced Training Programs and will contact the CHEK Institute by telephone or email to use the credit.
