This questionnaire was created
to help highlight the areas you may wish to include
in your next 12 months' action plan towards significance.
Below is the Annual Self Assessment(ASA) process diagram to be followed to arrive at your next 12 month Action Plan(AP).
The ASA consists of 3 Memory Prompts to assist you on arriving at your next 12 month AP and thereby moving you forward on your family's generational journey from Struggle, Stability, Success and Significance.
A completed and up to date 12 month AP is a requirement to be considered for any GIG loan.
PROMPT 1 – Family’s 7 and 21 Year Overview – Things to Do, Actions to Take
Prompt 1 - Family Structure and Goal Setting
Consider only the Immediate family related to You and Your Spouse (Self, Parents, Children and Grand Children). In some cases, you may have to assist with some Dependents (family or not). Add them into the Family Structure. These people are Your Emotional Reasons to do what you’re doing. If anything goes wrong in the lives of any of these people, it will spill over into You and Your Spouse’s Lives!
Once you have drawn out the Family Structure, put each person’s current age next to his name.
Then Add 7 Years to each person and write in the new age for each person. On a separate piece of paper write down the name of each person and next to this person’s name, jot down ALL the things that will have to be achieved or actions to be taken in this person’s life 7 Years from now!
Add 21 Years to each person’s Current Age and write it next to that person’s name. Repeat the exercise above for each person.
Prompt 2 – 8 Areas of Life From a Generational Perspective.
The 8 areas are:
1.Physical
This speaks of your physical location (the area you work and live in) and physical health (eating right and exercise)
2.Social Investment
This refers to your involvement in the community or church, being committed to a cause bigger than yourself to which you give time, energy and resources.
3.Wealth/Financial
This indicates how successful you are in terms of earning a good income (or even several income streams), be a regular saver, owning income producing assets or assets that protect you against inflation and having good investments. It includes having an investment strategy (even while You’re still in Struggle!) and actively work on it.
This is the sum total of growing personally through ongoing learning, reading,
attending seminars and courses and listening to positive talks and teachings
5.Business/Career
This speaks of being in your purpose, have a meaningful job or career, making progress, feeling rewarded and recognised for your achievements.
This is about having good relationships with your spouse and children, feeling supported in what you are doing, having good friends, and spending enough quality time together.
This is not about whether you are going to church regularly but whether you have developed a personal relationship with your Creator. Are you making personal time to get to know Him and learn about Him through His Word? It is not about rituals and works but about the relationship.In spending alone time with Your Creator, learn to listen in Your Spirit as He reveals Your and Your Family’s Purpose to You!
This refers to your hobbies, passions, having fun and enjoying life. This is about spontaneous laughter. This can also be about going on a retreat to a quiet place somewhere in nature and just find your sanity again.
Wheel of Life Exercise From a Generational Perspective
The next exercise is designed to look at each of these areas from a generational perspective and help you to calculate your own score for each of these areas.
Look at the question or statement under each of the headings and answer the questions that are True for you by ticking the checkbox. All the un-ticked boxes are an indication of area that need to be address at some point, preferably in the next 12 months.
Knowing what your wheel looks like and by going back to the questions, you will be able to counsel yourself into what to do next to restore the balance in your life. The pressures of the one generational world around us must not be underestimated and we must redo this exercise regularly to stay focused on our journey.

In concluding this Annual Self-Assessment (ASA) exercise we can learn from a historical prophet Micah:

The key character traits of a Generational Lifestyle can, therefore, be summarised as:
12m Action Plan
Using the various prompts of Your ASA, make a list of ALL the issues you wish to attend to during the next 12 months.
When setting up the list include the most important goals for you and your family in the areas of health, wealth, career/business, social responsibility, spiritual life, family and relationships, rest/relaxation/exercise/sport and intellectual growth.
With these goals in mind, create action steps to achieve them and put these action steps on your 12 month action plan.
