This form gathers information for MedMe to create new appointment types for your pharmacy. 

Please fill out one form for each appointment type request.

Enterprise Clients:
Please list specific store numbers for the required appointment type (separated by commas).

Co-booking works two ways. Service A can be booked with Service B and Service B can be booked with Service A, but you can state whether you don't want Service B to be co-bookable with Service A, if desired.

A flu shot can be added to a acne management appointment BUT 
An acne management appointment cannot be added to a flu shot

Please provide the "Next Steps" text that Patients will see in the booking confirmations and reminder emails for the appointment methods you've selected.

Example of Onsite Method - Next Steps:
  1. A day before your appointment, you’ll receive a text/email reminder.
  2. Please arrive 5-10 minutes before your appointment time, and let the pharmacist know when you arrive.

Example of Phone Method - Next Steps:
  1. 15 minutes before your call, you'll receive a text/email reminder.
  2. When it's time for your appointment, you'll receive a call at the phone number provided during your booking.
  3. Please make sure you are in a quiet environment or have headphones!

  1. 15 minutes before your call, you'll receive a text/email reminder.
  2. When it's time for your call, you'll receive a link to the video call, which will open up a tab in your browser. Alternatively, you may receive a call at the phone number provided during your booking.
  3. Please make sure you are in a quiet environment with working internet, speakers, and headphones!
