Formulario para ordenar materiales 

Thank you for your interest in sharing Medicaid Expansion educational materials with your community. Please fill out this form by January 8, 2024.  

Materials are free and orders will begin shipping in January 2024. Each order will include materials like printed bilingual flyers, coloring books, wallet cards and swag items.

Los materiales son gratuitos y las órdenes comenzarán a enviarse en enero de 2024. Cada orden incluirá materiales impresos como folletos bilingües, libros para colorear, tarjetas de billetera y artículos de botín.

If you do not see an appropriate audience, select "All Audiences." / ¿Tu organización representa o provee servicios a una de las siguientes poblaciones históricamente marginadas?
