Temporary Leave of Absence Request Form

Leave of absence endorsement gives a discount to the insured and interrupts the policy due to the insured’s temporary cessation of practice for a minimum of 30 days (not to exceed 12 consecutive months) in special circumstances. Coverage will not be provided for any claim or claims for injury arising out of direct patient treatment rendered during the leave of absence. This includes treatment provided via telemedicine.

 

A temporary leave of absence may not be available to those who participate in the patient compensation funds (PCF) in Indiana, Louisiana, and Pennsylvania. We will update this information once the PCF makes this information available.

 

Leave of absence is not available in Kansas except for military service

 

If temporary leave is due to medical reasons, a written release from your attending physician may be required to reactivate your policy. This statement should indicate fitness to practice and any limitations, and should be signed and dated by the attending physician.

 

A leave of absence will result in a change in premium. All policy changes are subject to Underwriting review and approval

Written notification must be provided if the end date or leave of absence changes from this original request date. It is the responsibility of the insured to notify PICA to request a return date change. Not doing so may impact coverage.
