Equipment Assessment Request Form

Our Assessment Process

The assessment will be conducted by our clinically trained product specialist and takes around 60 minutes.
Due to the technical nature of this equipment and the variations and adjustments often required we advise it is best to attend our facility in Mentone. We do have the ability to book appointments at the client's address when required.


It will include:

  1. Review of any current seating and discussion of needs with the Chair User / Client, allied health professional and/or the family and carers.
  2. Chair User / Client will be seated in the most suitable chair based on their measurements, postural needs and pressure care requirements. Please note we require a carer for any assisted transfers of the client into the chair.
  3. Evaluation of the Chair User / Client in the chair over a 30-45 minute period.
  4. Review and discussion of any further adjustments, options or customisations.
