The purpose of this form is to provide the required information to apply to deliver a TQUK qualification. If you require approval for more than one qualification you will have the option to submit another request upon submission.

Please refer to our qualification search for the full qualification title and number, and more information about our qualifications.

Before completing the form make sure that you've got everything you need to apply, this includes:
• the details of the staff that will be delivering the qualification
• the assessment that you intend to use to asses your learners
• any mapping documents you've created alongside the assessment.

Please note, this form can only be completed by a member of staff at a Recognised TQUK Centre with sufficient authority to request qualification approval.

Please note, additional qualifications can only be considered where the same circumstances apply across all the qualifications being applied for ie similar/same assessment methods, same assessment staff, and same DCS.
