BMES University Student Chapter Application Form

Thank you for your interest in starting your university’s BMES student chapter!

Please note, to qualify for compliancy status with BMES, your chapter will need at least 10 active BMES student members and 1 active BMES professional member as the faculty advisor. You will need their names, and emails to complete this application, so please be sure to have that as you begin.

An active BMES membership means the students and the advisor listed ALL have either paid or renewed their student or professional membership with our society. Expired, nonmember and/or unpaid memberships will not qualify .

For joint universities renewing or starting a BMES student chapter, you will need at least 15 active BMES student members and 2 active BMES professional members as the faculty advisors.

Please note if your application does meet the BMES membership requirements above, it will not be accepted.

Role in the Chapter*
List all members of your chapter and their emails in the space provided below. Remember to be considered, you will need at least 10 active BMES student members to proceed.

If your roster includes chapter’s officers, please note their positions.

If your chapter has over 25 members, you do not need to list all, please cap your roster list to 15 or less for single universities and 25 or less for joint universities.
