Gold Licence Additional Application Form - Credit/Debit Card

Gold Licence Applications are submitted to the Caterers Accreditation Council of Restaurant & Catering Australia.

If you have multiple additional businesses, you will need to fill out the form again for EACH additional business.

Please note that the accreditation process may take up to four (4) weeks.

Please ensure all supporting documentation is included with your application (see section 2 for the checklist). The application will be reviewed by the Caterers Accreditation Council only when ALL supporting documentation is supplied.

Health Inspection Report – last one conducted
Public Liability Insurance
Appointed First Aid Officer
Workers Compensation Insurance
Take Away And Food Delivery (if required) please send photos of containers
Food Safety and HACCP Plan
Do you have food transportation vehicles? Please send photos of vehicles inside and out
Food Safety Supervisor Certificate
Other Commercial Kitchen Premises (if required)
Business Liquor Licence (if required)
Industry Experience such as resume

By signing this we acknowledge any evidence showing the business has received an infringement that is note notified to the Caterers’ Accreditation board, means the accreditation can be removed.

I /We declare in the name of the applicant, all supporting documents have been submitted and include current employee's/subcontractor's details that relate directly to the business.
