If you use your own cover letter for the shipment, please make sure to include the reference number you submitted in the form.

NOTE: Only fill this form if you wish to send in your equipment for repair or service.
Customers from countries outside of the EU must enclose a pro forma invoice with the repairs, as this is required for customs clearance in Denmark. The pro forma invoice should include a value for each unit, the customs tariff code 90328900, and a remark stating that the equipment is sent to OxyGuard for repair and will be returned to the customer after repair.
