This form must be completed in order to disable by default, for all appointments at a given institution, the checkbox for patients to receive automated appointment communications (appointment booking/change/cancellation notices, appointment reminders).

By completing this form, you confirm that you are authorized to make this request on behalf of your institution.

By checking this box, I agree to disable automated appointment communications by default for all appointments in my institution. It is possible to send appointment communications by checking the box in the appointment window for each appointment.
