If you have chosen a full or partial distribution we will need you to complete a Distribution Form. If you do not have a bank account on file, we will also need you to complete an Electronic Funds Transfer authorization form. Upon completion of this Maturity Form, you will receive an email confirming the option you selected above, as well as links to the needed forms.
